Exemplar Health Care, a leading provider of specialist nursing care for adults living with complex and high acuity needs, has now opened a new £5.7m care home in Sunderland. The official opening follows a formal registration visit by the Care Quality Commission, the regulator of health and care services in England.
Hylton Grange specialises in supporting younger adults living with dementia, acquired brain injuries, complex mental health conditions and physical disabilities.
Split in to three, ten-bed communities – as small group living provides a more homely, supportive, and responsive care environment – each of the 30 bedrooms has its own ensuite wet room. The home also features spacious communal and activity rooms, sensory and therapy rooms, and landscaped gardens, ensuring that residents can enjoy a full and varied lifestyle.
Having created over 100 new jobs in the local area, Hylton Grange benefits from an in-house multidisciplinary team of Health Care Assistants, Registered Nurses, an Occupational Therapist and a Physiotherapist. This team will work together to ensure that residents receive the specialist nursing care they need, enabling them to stay closer to their loved ones. Without the specialist home, residents would otherwise have a pro-longed stay in hospital.
The new staff team, having completed a four-week bespoke induction, prides itself on providing person-centred care, empowering residents to maximise their independence in a way that works for them based on their healthcare needs, and providing the support they need to live fulfilling and enjoyable lives.
Several roles are still to be recruited for including a Registered Nurse and a Senior Nurse Unit Manager, with both roles paying a welcome bonus of £1,500.
